Case management includes communication with the individual and family, and part of case management is care coordination with the physician, family, and community resources.
Each assessment includes plan for education as well as availability of a nurse for phone consultation 24/7. In addition, specific services such as medication set up or completing non-skilled tests in the home such as INRs are available.
