Teaching and Managing Chronic Conditions

Management of heart failure and Chronic Obstructive Pulmonary Disease (COPD) by Dove nurses

Heart Failure and Chronic Heart Failure (CHF):

  • Education for heart failure and CHF includes a focus on how to recognize when the symptoms of Heart failure and CHF are well managed or when it is time to discuss worsening symptoms with a nurse or doctor.

  • Heart failure or CHF symptoms include shortness of breath (dyspnea), chronic cough or wheezing, edema or swelling, high heart rate, and confusion or impaired thinking.

  • Education includes tools to help manage the symptoms of CHF and how to recognize when the disease is well managed or worsening and when to call our team of experienced nurses or physicians.

  • Although CHF can’t be reversed, the education and training provided in-home by the Dove team of experienced nurses can help clients to understand the medications prescribed and when to use them to help minimize further damage to the heart. Dove works closely with physicians to ensure symptoms are well managed.

Chronic Obstructive Pulmonary Disease (COPD):

  • COPD is used to describe progressive lung disease and may include emphysema, chronic bronchitis, and non-reversible asthma.

  • Symptoms of COPD may include chronic cough, breathlessness or shortness of breath, and wheezing.

  • Education includes tools to help manage the symptoms of COPD and how to recognize when the disease is well managed or worsening and when to call our team of experienced nurses or physician.

  • Chronic Bronchitis and Emphysema are the most common conditions that makeup COPD.  Although COPD can’t be reversed, the education and training provided in-home by the Dove team of experienced nurses can help clients to understand the medications prescribed and when to use them to help minimize further damage to the lungs.
